On Friday, April 28, 2023, a teacher and staff member of a Kobe Municipal Elementary School lost materials and other documents containing names and other personal information of children.
1. Lost documents, etc.
A list containing the names of all students at the school (230 names)
Test answer sheets for three classes of 3rd and 4th graders (64 students)
List of names of students for checking answers (for 69 students)
Address lists (including telephone numbers) of teachers and staff of the school for the last school year and the school year before last (35 names)
KIIF terminals (PCs used by teachers and staff for office work; third parties are not allowed to log in)
2. Background and response
A teacher of the school took the lost documents and other materials out of the school in his bag to work at home on his day off.
After eating and drinking with the staff, he realized on his way home that he had lost the bag. On April 28, he searched the bag by himself, but could not find it.
On the following day, April 29, he searched the bag again, but could not find it. He reported the lost bag to the police and also to the management.
On May 1, a meeting was held for parents to explain the incident and apologize to the parents.
3. Efforts to prevent recurrence
The importance of protecting personal information and the handling of personal information when it is taken out of the premises will be reiterated.
