Kobe City: Mimamori Sticker

Project Summary

This sticker enables prompt identification and protection of elderly persons with dementia, etc. when they go missing.

• When a person who has found the sticker reads the 2D code on the clothing, etc., the city website will be displayed.
• Report the seal number and current location to the call center listed on the website
• The call center immediately confirms the identity of the person and contacts the police for prompt protection.
• To protect personal information, only the registration number is written on the seal.

Fee

Free (20 stickers distributed per person)

How to apply

1. Visit your local Anshin Sukoyaka Center
2. Apply at the same time as the Senior Citizen Safety Registration Program

Required documents

• If you have not used the Dementia Kobe Model, submit a medical certificate (prescribed form)
• The medical certificate form can be obtained from here (PDF: 99KB )

If it is difficult for the applicant to come to the center, please consult with the center in advance.
