The Vaccination Certificate
- is located on the right side (for the 1st and 2nd vaccinations) or left side (for the 3rd, 4th, and 5th vaccinations
) of the vaccination ticket.
- Please check your vaccination ticket.
- Only those who have a certificate of residence in Kobe City as of the date of vaccination can have their Certificate of Vaccination reissued
.
- If there is no vaccination record in the vaccination record system, it may take several months to issue a reissue of the vaccination certificate.
