How to apply and where to apply

Please consult and apply at the Health and Welfare Section of your ward office or Tamatsu Branch Office (Health and Welfare Service Counter ).

What you need to bring with you when you apply

• Application form for Rehabilitation Certificate (available at the counter when you apply)
• Photo (upper body, 4 cm (length) x 3 cm (width)) (upper body, 4 cm (length) X 3 cm (width))
