The following is the application procedure for those who are eligible for Kobe City Colorectal Cancer Screening and meet all of the following criteria.

• Age 40-69
• Have insurance other than Kobe City National Health Insurance
• Have a free examination ticket, etc.

Flow of Examination

1. Application
   • Application period: Sunday, October 1, 2023 – Thursday, February 29, 2024
2. Test kit is delivered to your home
3. Collect stool samples using the test kit (2 days)
4. Mail the test kit
   • Submission period: November 1, 2023 (Wednesday) ~ March 15, 2024 (Friday) (must be postmarked)
5. Notification of test results
   • Test results will be mailed to your home approximately 3 weeks after the test kit is mailed.

Notes

• Even if you apply in October, the test kit will not be sent until November.
• If you change your address after applying, please contact the Hyogo Preventive Medicine Association.

For those who have a 40-year old comprehensive medical checkup ticket

Please mail the white envelope enclosed in the 40-year-old comprehensive health checkup ticket.

Those who belong to “households in which all household members are exempt from city tax or taxed only on a per capita basis”

Please obtain a free medical checkup ticket and send it to Hyogo Preventive Medicine Association.
